Your Work Environment You’ll be part of the therapy specific team supporting Home and Heart Home Health Care Agency, working within a collaborative, multidisciplinary model. Care is coordinated, patient‑centered, and team‑driven, with shared accountability and open communication. Job Overview & Work Site Your Patients You’ll work with a diverse caseload, including a higher‑than‑average post‑operative orthopedic population. How You’re Supported You’re set up to do your best work through: Thoughtful scheduling and defined territories to support appropriate caseloads Limited drive time between visits whenever possible Clinical leadership from therapists who understand day‑to‑day practice demands Individualized support aligned with your clinical interests and goals Collaboration with Home and Heart healthcare colleagues for a team‑based approach to care

Requirements

  • Graduate (current or pending) of an accredited occupational therapy program
  • Active or pending state occupational therapist license
  • New graduates encouraged to apply
