Requirements

  • Graduate of an accredited Occupational Therapy program
  • Current, unrestricted Occupational Therapist license in the state of practice
  • Minimum 1 year of experience in a clinical care setting
  • Ability and willingness to travel within the assigned coverage area (approximately 50%)
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to lift, push, pull, and support up to 50 pounds, including patient transfers and equipment movement

Responsibilities

  • Evaluate patients to determine rehabilitation needs and develop individualized plans of care
  • Perform initial evaluations and ongoing reassessments according to OASIS and PPS regulatory guidelines
  • Implement and adjust treatment plans to ensure quality outcomes and continuity of care
  • Communicate patient progress, significant changes, or concerns to the physician and care team
  • Educate patients and caregivers on activities of daily living (ADLs), adaptive equipment, and home safety
  • Assist patients and families in obtaining necessary equipment, including custom splint fabrication when appropriate
  • Provide instruction and support for home therapy programs
  • Collaborate with interdisciplinary teams to ensure the highest quality patient care
  • Supervise and provide clinical guidance to Certified Occupational Therapy Assistants (COTA), Home Health Aides (HHA), and other clinical staff
  • Ensure documentation and care delivery meet professional standards and regulatory requirements
