About The Position

Requirements

Bachelor’s or Master’s Degree in Occupational Therapy from an accredited program
Active Texas Occupational Therapist (OT) license
Home health occupational therapy experience preferred (1+ year required)
Experience with OASIS documentation preferred
Current CPR certification
Strong communication, clinical judgment, and time-management skills
Knowledge of home health regulations and Medicare guidelines preferred
Valid driver’s license, reliable transportation, and current auto insurance

Responsibilities

Perform home health occupational therapy evaluations, including OASIS assessments
Develop and update individualized Plans of Care (POC)
Provide skilled home health occupational therapy for orthopedic, neurological, cognitive, and post-surgical conditions
Address ADLs, IADLs, upper extremity function, coordination, strength, balance, and functional mobility
Implement interventions for fall prevention, home safety, and environmental modifications
Train patients and caregivers in adaptive equipment and durable medical equipment (DME)
Fabricate splints and recommend assistive devices as appropriate
Complete accurate and timely documentation in the EMR
Communicate with physicians and interdisciplinary care teams
Supervise COTAs and home health aides, as applicable
Follow all home health regulations, infection control, and safety standards
