About The Position

Human Touch Home Care is looking for an Occupational Therapist to join our team. We value our employees like family. We are a locally operated home care company that has been helping our clients to live their most healthful and fulfilling lives at home in Grand Junction (Mesa County) . We are seeking someone to provide skilled Occupational Therapy to patients in their homes in accordance with the physician's orders.

Requirements

• Bachelor's degree required.
• Minimum of one year of experience as Occupational Therapist .
• Must be license as an OT in the State of Colorado
• Must have reliable transportation and company-required auto liability insurance.
• Willing to travel within 20 miles radius.
• CPR Certified.
• Ability to commute: Grand Junction, CO: Reliably commute or planning to relocate before starting work (Required)

Nice To Haves

• Home health experience preferred.

Responsibilities

• Provides services in Occupational Therapy support in accordance with the physician's plan of treatment. Accepts only clients referred by the Administrator/DON and/or case manager
• Assists the physician in assessing the client's level of functioning and development (and revision, as necessary) of the plan of care by applying appropriate tests for occupational therapy
• Records and reports the client's reaction to treatment and any changes in the client's condition to the physician, Administrator/DON and/or case manager
• Instructs and advises the client, family, and other health team personnel in the phases of occupational therapy in which they may assist the client
• Recommends additional care and treatment when necessary
• Participates in once weekly case conference or in-service
• Conducts LPN and CNA supervisory visits as assigned
