What You’ll Do At Denver Human Services (DHS), we envision a healthy community where people are connected, supported, safe, and well. Our employees help children, older adults, families, and individuals navigate social and economic pressures by connecting them to services and experts who support their overall well-being. Denver Human Services Engagement and Outreach Division is currently seeking a dedicated, well-versed, and experienced professional to serve as an Outreach Case Coordinator II on our Outreach team. This team will specialize in providing one-on-one services to customers, using a holistic, goal-oriented approach that is driven by the customer’s identified needs and goals. This team is designed to assess customer needs, facilitate coordination between city agencies, DHS departments, social service systems, and community partners. The goal of this Team is to serve customers that typically have a hard time accessing services where their needs are not currently met and require a more direct hands-on approach service to increase self-sufficiency, stability, and quality of life, in a manner that respects customer- identified goals. The outreach team can meet with customers at all DHS locations and provide outreach services to meet with customers in the community. Additionally, the Outreach Case Coordinator II will: Work independently in diverse environments and will be available to work weekends and evenings when needed Travel to individual locations, other agencies, and community resources for the purpose of arranging and providing services for individuals, and families Make referrals to other public and private social services and community agencies and resources to meet individual and family needs, assists individual and their families in understanding and utilizing these resources Performs outreach, presentations and in-service training sessions to outside service providers, vendors, community organizations, program partners, and/or other counties to ensure the ongoing building of knowledge about our program. Engages in ongoing coaching, problem-solving and facilitation with clients to resolve immediate or recurring problems and barriers to receiving and/or participating in services or programs, This position may be re-deployed to work in other capacities in their own agencies or in other City agencies to support core functions of the City during a City-wide emergency declared by the mayor. Our ideal candidate has some or all the following experience, skills, and characteristics: Professional experience offering peer navigation, case management, resource referral, and navigating systems including Human Services, Justice/Courts, Health Systems, Shelter and/or transitional housing Strong relationship building and networking skills. This role will rely heavily on community resources/partners and need to navigate internal (to DHS) and external resources to deliver appropriate services for each customer situation or presenting need Strong customer service both internal to an agency and customer facing, preferably with skills in problem-solving, analytical thinking, motivational interviewing, de-escalation and coping skills, and experience administering trauma informed practices Excellent and diverse communication skills. The selected candidate will be comfortable working one-on-one with clients Understanding of General Assistance and Public assistance programs, community resources in Denver and surrounding areas Strong organization, accountability, and case documentation experience Bilingual in Spanish and English (plus, but not required) Self-motivated and able to work virtually as needed Experience with Microsoft teams and other computer skills Required Minimum Qualifications Education requirement: Graduation from high school or possession of a GED, HiSET or TASC Certificate. Experience Requirement: Two (2) years of experience performing intake including assessing and identifying client strengths and needs and providing referrals for assistance. Education/Experience Equivalency: Additional appropriate education may be substituted for the minimum experience requirement. Licensure: Requires a valid Driver's License at the time of application.
