About The Position

Franciscan Medical Group, as part of Virginia Mason Franciscan Health, is currently seeking a full time Outreach Peer Navigator for our busy Franciscan Family Medicine Residency Clinic in Bremerton, WA. No weekends or organizationally recognized holidays required. Job Summary: This job is responsible for serving as a patient advocate for the Virginia Mason Franciscan Health (VMFH) grant-funded program focused on increased routine HIV, Hepatitis C and Syphilis screening and linkage to treatment and care. Focused on the most vulnerable populations, work is accomplished through effectively locating patients in the community, building relationships, coaching, patient education, and planned follow-up activities. The Navigator will provide guidance and education to the patient, working to eliminate barriers for effective linkage to ongoing care, as well as facilitating access to support services, community resources and primary care, reinforcing the need for active self-management in maximizing overall health.

Requirements

  • Associates Other -Accredited AA Degree with emphasis in social services, psychology or related health field preferred.
  • At least two years' experience in health care, social service setting including any experience conducting community outreach, case management, or patient focused services preferred.

Responsibilities

  • Facilitate care coordination, playing a vital role in locating and contacting patients in the field, connecting patients with resources and supporting them in navigating the healthcare system by addressing barriers to care access without providing clinical care or substituting for services offered by doctors, nurses, or social workers.
  • Establish positive relationships with patients utilizing exceptional listening, verbal and written communication skills, as well as providing culturally and linguistically appropriate health education to access and complete STI treatment, promote physical and mental health, and prevent reinfection or transmission of disease.
  • Serve as a liaison between patient and care team, staff, community resources, and others, and support patients in learning to navigate the healthcare system independently
  • The role promotes disease prevention and physical and mental well-being by leveraging organizational and community resources and services in Kitsap County, and connecting patients to those resources both within and outside the organization.
  • The role is responsible for building and maintaining a positive working relationship with providers, community-based organizations, social services, and public health contacts, and must collaborate with healthcare teams and may require meeting with community leaders to assist in the process of building and maintaining a working/business relationship between the team and community partner.
