Role Scope: We are looking for a sales- oriented healthcare professional who is passionate about serving the underserved community. The Outreach Specialist will educate and build relationships with community partners, prospective participants, families, and/or caregivers to create awareness about Habitat Health’s PACE (Program of All-Inclusive Care for the Elderly) program. Ultimately, the Outreach Specialists create referrals or direct requests for enrollment, enabling the frail elderly to live independently at home and continue to enjoy an optimal quality of life. In addition to outreach and lead generation responsibilities, this role will support Enrollment Specialists by performing enrollment-related tasks such as making outbound calls and following up with referrals as needed.

Requirements

  • Experience in healthcare or enrollment sales, with a track record of generating business-to-business or business-to-customer referrals
  • Experience working with State/County eligibilities workers
  • Proficiency in CRM software preferred
  • Experiences working with Frail Elderly
  • Highly motivated individual with minimal supervision
  • Excellent verbal and written communication skills, with the ability to build rapport and deliver clear, compassionate messaging.
  • B.S./B.A. Degree in healthcare, Business Administration, or Marketing preferred
  • Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment
  • Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law
  • Must be medically cleared for communicable diseases and current on all immunizations prior to participant interaction (including TB clearance)

  • Bilingual: Spanish/Russian/Vietnamese/Mandarin/Cantonese preferred.

Responsibilities

  • Develop and maintain relationships with healthcare professionals, providers, Community-based organizations, senior housing facilities, post-acute facilities, and senior centers to drive referrals for the Habitat Health PACE program
  • Collaborate with the Enrollment Specialist to ensure successful enrollment
  • Follow up with prospective participants and referrals through outbound calls, home visits, and ongoing communication to move them through the enrollment pipeline
  • Organize and conduct events in the community, and conduct presentations to promote the Habitat Health PACE program
  • Maintain and record all productions/activities in CRM software
  • Strictly adhere to DHCS and CMS regulations and policies regarding marketing practices for the PACE program
  • Attend various community events, healthcare events, and increase the center’s census
  • Identify new opportunities to enhance and improve the referral process and activities
  • Flexible to working events in the evening and weekends as needed
  • Other duties as assigned
