About The Position

AASC’s PACE Program strives to provide services to frail older citizens who meet the criteria for nursing home eligibility, but opt to remain at home with supportive services provided through the PACE Program. The PACE Center Manager, working closely with the PACE Management Team, Medical Director/Clinical Staff, Therapy Staff, Day Center Staff, Transportation Staff, MSW and Home Care Staff will be responsible for the delivery of all day-to-day PACE Center services. The PACE Center Manager is a liaison between PACE disciplines, departments, external providers and families/caregivers.

Requirements

  • Minimum Bachelor’s degree or equivalent experience. Master’s degree preferred.
  • 5-7 years of supervisory management experience required with demonstrated leadership and interpersonal skills.
  • A minimal of two years’ experience working with older adults, and persons with disabilities required.
  • Must have a valid driver’s license, pass a criminal background check and be medically cleared of communicable diseases.

Responsibilities

  • Directs and oversees all PACE Center services, ensuring appropriate and effective service delivery.
  • Ensures compliance with local, state and/or federal mandates and guidelines, or other regulatory agencies. Recommends policy to management and implements accordingly based on guiding principles and standards.
  • Complete program reports and submission to appropriate entities as required.
  • Exhibits complete confidentiality at all times in all matters in regard to all program participants, their families/caregivers by observing all relevant Federal and state laws, rules and regulations and all ethical rules of conduct.
  • Actively recruits program participants and oversees the completion of appropriate program documentation.
  • Oversees orientation, training, and scheduling of all PACE Center staff, interns and volunteers.
  • Develops resources to meet program needs.
  • Oversees development of participants’ plans of care by the interdisciplinary team (IDT) by facilitating the IDT Care Plan meetings and Morning Meetings.
  • Monitors and tracks service interventions noted in the Plan of Care.
  • Reports any suspected abuse, neglect or exploitation and reports to the appropriate Department of Social Services.
  • Serves as a member of the AASC Management Team, PACE Interdisciplinary Team, Quality Improvement Committee and other appropriate committees as assigned.
  • Assist with Service Delivery Requests, Grievances, and Appeal processes when warranted.
  • Perform other duties as directed by CEO.
