About The Position

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: Monday-Friday 8:00am-4:30pm Position Summary: The PACE Certified Nursing Assistant (CNA) provides personal care and supportive services to participants in the PACE Center, home, hospital, nursing home, or assisted living facility. Care is delivered under the direction and supervision of a Registered Nurse, PACE Center Director, or Home Care Manager.

Requirements

  • At least 18 years of age with a high school diploma or General Education Degree (GED).
  • Currently registered as a State Nurse Aide through the Kentucky Board of Nursing.
  • Completion of a recognized Certified Nursing Assistant (CNA) training program.
  • Minimum of one (1) year of experience working with frail and elderly population.
  • Food handling permit required (to be obtained upon hire).
  • Maintains current CPR certification.
  • Self-directed with the ability to work independently with minimal supervision.
  • Demonstrates knowledge and skills required to provide care to and communicate effectively with the adult and elderly population.
  • Ability to read and interpret safety rules, operating and maintenance instructions, and procedural manuals.
  • Ability to write routine reports, documentation, and correspondence accurately.
  • Ability to work collaboratively with PACE staff and members of the Interdisciplinary Team (IDT).
  • Ability to communicate effectively with participants, families, and employees, including speaking before small groups as needed.
  • Ability to understand and carry out written, oral, or diagram-based instructions.
  • Ability to solve routine problems involving multiple variables using standardized procedures.
  • Ability to adapt to changing participant conditions and care needs as directed.
  • Ability to accommodate daily schedule changes related to staffing needs, participant status, and new participant admissions.
  • Ability to follow emergency, safety, and evacuation procedures.
  • Must be a licensed driver.
