About The Position

The Program of All-Inclusive Care for the Elderly (PACE) is a community-based program that provides coordinated medical and social services to eligible older adults who want to continue living in their own home despite chronic care needs. The Driver, under direct oversight of the Transportation Manager, is an integral member of the PACE at Hudson Headwaters (PHH) team. The Driver provides safe and reliable transportation for Participants to get to and from the Day Center, medical appointments, and delivery of medical supplies and/or medications. The Driver is responsible for communication and interaction with Participants, Participant families or caregivers and with all Interdisciplinary Care team (IDT) members.

Requirements

  • High School Diploma or general education degree (GED)
  • Current BLS certification or willing to complete certification
  • Requires personal and reliable transportation, current state-issued driver’s license, and clean driving record
  • Have 1 year of experience working with a frail or elderly population or be willing to receive appropriate training upon hire
  • Demonstrated ability to work collaboratively and to maintain working relationships and team building within agency and with outside partnerships
  • Ability to communicate clearly and effectively
  • Able to work positively in a diverse and stressful environment
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards
  • Ability to lift heavy equipment
  • Will require 20% sitting, 30% walking, and 50% standing
  • Load, tie down, and maneuver wheelchairs
  • Driver will have to stoop, kneel, crouch, lift, and stand for long periods
  • Will require exposure to a variety of weather conditions including snow, rain, cold, etc.

Responsibilities

  • Transport program Participants to and from the Day Center or other assigned areas, as directed by the Transportation Manager or designee.
  • Assist the Participants to and from the vehicle to the Participants' residence or the Day Center in a safe manner, using the appropriate assistive devices as necessary, proper body mechanics and following all infectious disease policies.
  • Participate in all scheduled education programs, in-services, or staff meetings as directed.
  • Utilize safe and appropriate body mechanics, and transfer techniques when providing transportation services to Participants.
  • Responsible for properly securing wheelchairs and safety belts for wheelchair Participants.
  • Maintenance of the vans—adding oil to the motor, adding brake fluid, windshield wiper fluid, brake fluid, transmission fluid, putting air in tires, charging battery if van won’t start, as well as gassing van and dropping off/picking up van for repair.
  • Cleaning of vans—this includes extermination & disinfection of van after contamination of bed bugs and bodily fluids such as vomit, urine, and feces.
  • The delivery of medications, meals, and supplements such as Ensure, frozen meals, incontinence products, etc.
  • Assist with picking up any items that belong to PHH or any items the family may choose to donate from a Participant residence.
  • Assist with ensuring Participants’ take-home supplies are brought home and safely arrive inside the Participant residence.
  • Understand and adhere to PHH policies and procedures and PHH's Participant Rights Policy.
  • Protect privacy and maintains confidentiality of sensitive Participant and PHH information.
  • Attend care planning meetings as requested, and in-services and education programs as required.
  • Report any variance, accident, or unusual occurrence to the appropriate personnel.
  • Additional work functions and duties may be assigned.
