About The Position

JOB SUMMARY: Plays a vital role in supporting the PACE clinical team. This nurse is responsible for the comprehensive review and analysis of participant health records – including medication administration records, laboratory results, and consult reports from network providers – with a focus on ensuring safety, appropriateness, and effectiveness of care for frail elders. The nurse reports key clinical information to medical providers to support informed care decisions. This nurse will proactively monitor prescriptions, ensuring timely renewal and notifying providers of upcoming expirations. Regular communication with participants, caregivers, and pharmacies is essential to reconcile medication lists, provide updates on treatment plans, and ensure continuity of care.

Requirements

  • Graduate of an accredited school of nursing and be licensed by the Virginia Board of Nursing as a registered nurse
  • Minimum of two years of clinical nursing experience, preferably in geriatrics, long-term care, care coordination, or case management
  • One or more years’ experience with a geriatric population
  • Proficient in reviewing clinical records interpreting medical data and understanding geriatric pharmacology
  • Experience with EHR systems and documentation
  • Understanding of chronic disease management, polypharmacy, and age-related care needs
  • Medically cleared for communicable diseases with up-to-date immunizations before engaging in direct participant contact.

Responsibilities

  • Review and interpret medication administration records, lab results, and external consult notes for geriatric participants
  • Identify and communicate clinically relevant information to PACE medical providers
  • Monitor prescriptions for upcoming expirations and ensure timely renewal with prescribers
  • Coordinate with contracted pharmacies to reconcile medication lists and ensure alignment with prescribed treatment
  • Assist in management of upcoming procedures including arrangement and coordination for pre- and post-op needs
  • Communicate changes in treatment plans or medications clearly to participants and caregivers
  • Support education efforts for participants and caregivers as needed
  • Occasionally acts as a clinic or field nurse providing direct participant care
  • Provides nursing care according to accepted practice standards
  • Respects participants’ rights and treats patients in accordance with ethical standards.
  • Complete all documentation in the medical record and/or electronic health record (EHR) as required
  • Acts only within the scope of his/her authority to practice
