The Family Peer System Navigator program works with youth and families to ensure access and enrollment into services and resources offered by the child serving system in Delaware County. The Family Peer Specialist (FPS) uses their lived experience to engage with youth and caregivers and supports them in accessing appropriate supports and services to meet the youth’s complex emotional and behavioral needs. The FPS learns about the family and their needs and provides education and information on how to access the appropriate services and resources that match the needs of the family. The FPS works with the family for 3-6 months or until the youth and family are enrolled in the appropriate services and resources. The Delaware County Family Peer System Navigator, Family Peer Specialist, will work with youth and families being served through the project, providers, and county staff in Delaware County. The Family Peer System Navigator takes a System of Care approach to care coordination that looks to ensure the “no wrong door” standard, as well as a youth and family driven process. The program is an extension of High-Fidelity Wraparound and will share key constructs such as Family Voice and Choice, Individualized care, natural supports, integrated planning, strengths-based approaches. Please note, that in order to be a Family Peer Specialist, it requires you to have had personal involvement either yourself, or as a parent or caregiver with a child, involved in mental health services.
