About The Position

The Patient Care Supervisor is responsible for overseeing evening hospice operations to ensure high‑quality, patient‑centered care. This role provides clinical leadership, triages calls, coordinates after‑hours visits, supports field staff, and ensures continuity of care for patients and families. The Patient Care Supervisor serves as the primary resource for clinical concerns, care planning decisions, and patient needs after regular business hours. This position will work from 3 - 11:30 pm, Monday - Friday and will be based in our Fargo office location.

Requirements

  • ND and MN, unrestricted Registered Nurse (RN) license. (or willing to obtain dual licensure after hire)
  • Active driver’s license in good standing.
  • Minimum 1–2 years of nursing experience, preferably in hospice, palliative care, or acute/critical care.
  • Strong triage skills and ability to make quick, sound clinical decisions.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work independently and manage multiple priorities.
  • Experience with EMR systems commonly used in hospice care.

Nice To Haves

  • Bachelors in Nursing, preferred.
  • CHPN (Certified Hospice & Palliative Nurse) certification, preferred.
  • Previous leadership or coordination experience.

Responsibilities

  • Follows company policies to ensure consistent and compliant job performance.
  • Performs all other duties as assigned.
  • Provide real‑time coordination of hospice and other organizational program services to ensure timely, appropriate care.
  • Triage patient, caregiver, facility, and provider calls; determine urgency and dispatch clinical staff as needed.
  • Evaluate clinical information to support decision‑making around symptom management, medication adjustments, and care planning within scope and protocols.
  • Communicate significant changes in patient condition to the on‑call provider, interdisciplinary team, and leadership when appropriate.
  • Coordinate non-medical transport as appropriate.
  • Serve as the on‑shift clinical resource for RNs, LPNs, Chaplains, Social Workers, and CNAs.
  • Support staff by answering clinical questions, guiding symptom management strategies, and ensuring compliance with hospice standards.
  • Provide mentorship and promote a culture of compassion, professionalism, and teamwork.
  • Review and ensure accurate, timely, and complete documentation in the EMR for all patient encounters.
  • Maintain compliance with Medicare Conditions of Participation, state regulations, and organizational policies.
  • Support quality initiatives, incident reporting, and follow‑up actions as needed.
  • Coordinate shift handoffs with the day team and night on‑call team to ensure continuity of care.
  • Monitor caseloads, staffing needs, equipment requests, and medication orders.
  • Collaborate with pharmacy, DME vendors, and partner facilities to ensure seamless care delivery.
  • Serve as a point of contact for patients and families regarding care needs, emotional support, and education.
  • Provide compassionate guidance around hospice philosophy, symptom management, and end‑of‑life expectations.
