Acts as a liaison between Shoreline Behavioral Health Services and Tidelands Health Emergency Department to coordinate services for emergency room patients identified as having a substance use disorder. Provides and coordinates referrals, assists clients, provides health education to clients on topics such as substance use and provides case management services. Documents all attempts to link clients to care, conducts surveys at prescribed intervals, facilitates transportation for clients as needed and works closely with both Shoreline and emergency department staff. NOTE: This is a full-time, grant-funded position, and will be located at Shoreline Behavioral Health Services in Conway initially but will eventually be located full-time in the Tidelands Health Emergency Department. Applicant must be available to work on Sundays.
