We serve older adults and adults with disabilities in the community. The Registered Nurse serves as an integral piece of our agency ensuring compliance with the Oregon Administrative Rules.

Requirements

  • Active Oregon Registered Nurse (RN) license in good standing
  • Minimum two (2) years of professional experience working with older adults, adults with disabilities, dementia, or individuals with mental health conditions
  • Demonstrated experience in medication management, caregiver and client assessment
  • Strong written and verbal communication skills, including professional documentation and clinical note-writing
  • Proficiency with Google Workspace (Docs, Sheets, Gmail) and electronic care management platforms (WellSky experience preferred)
  • Excellent critical thinking, problem-solving, and organizational skills
  • Ability to work independently in community settings while maintaining strong team communication
  • Valid Oregon driver's license, reliable personal transportation, and ability to pass a Motor Vehicle Records (MVR) check
  • Pass a criminal background check in accordance with Oregon OAR 407-007-0000 and agency policy
  • Pass a pre-employment drug screening consistent with Oregon law
  • Provide proof of current CPR/First Aid certification (or obtain within 60 days of hire - training provided)
  • Provide documentation of valid Oregon professional license or registration
  • Provide proof of valid Oregon driver's license and current auto insurance

Nice To Haves

  • Experience working Veteran's and older adults with mental illness
  • Familiarity with Oregon's aging and disability services network, including APD, AAA, and community mental health programs

Responsibilities

  • Clients - Medication Management Conduct thorough initial assessments of client health and medication regimen.
  • Fill medsets monthly and bi-monthly Responsible for tracking new orders and refills of medication Liaison between doctors, clients, and responsible parties to ensure medications are delivered safely and appropriately
  • Clients - In Home Care Conduct thorough initiaion assessments and quarterly assessments for clients receiving in home care services Ensure medication admininstration is performed accurately with signed doctor orders Delegate necessary nursing tasks in accodance with signed doctor orders Participate in quarterly quality improvement meetings as required under the Oregon Administrative Rules
  • Caregivers - Evaluation and Training Conduct initial caregiver competency evaluations of new hire caregivers and CNA's to ensure their capacity to perform caregiver tasks Conduct initial medication competency evaluations of new hire caregivers and CNA's to ensure their capacity to administer medications Delegate necessary nursing tasks in accordance with caregiver's ability to perform tasks Provide additional training to caregivers as necessary
  • Documentation & Compliance Maintain accurate, timely, and detailed documentation of all services rendered using the WellSky Home Care portal Adhere to all applicable Oregon state regulations, HIPAA requirements, and agency policies regarding client confidentiality Perform other duties as assigned by the Director of Care Services
