About The Position

JOB SUMMARY The Nurse Pra c titio n er is a registere d pro f essiona l nurse w i th advan c ed graduate education and clinical training who p r ovi d es c omprehensive hospi c e s ervi c es in compliance wit h the s tate Nurs e Pr a ctice Act and under the supe r vis i on of a colla b orating phys i cian. The Nurse Pra c tit i oner p r ovides hosp i ce c are through the evalu a tion an d asses s me n t of the health h isto r y, hea l th promot i on, d i agnosing medical condition s , management of health p r oblems by di r ecting and deve l oping the pl a n of ca r e, pr e scribing medic a tions or treatments , coordinating and collabor a ting with pat i ents, f a mili e s, and othe r healthca r e practitio n ers. Under the s u pervis i on o f a phys i cian, t h e nurse pra c tit i oner visits p atients, compl e tes physical assessments , per f orms face to face a s sessments and evaluate s hospi c e eligibility as assigned in consultat i on with the hosp i ce interdi s ciplinary te a m. The nurse pra c titio n er builds from t h e r e sour c es of the communi t y to plan an d direct serv i ces to meet th e need of individuals and f a milies within t h eir homes and communities.

Requirements

  • Graduation f r om an a c cre d ited School of Nursing and accredite d Nurse Pract i tio n er P r ogr a m.
  • Master's degr e e w i th a mi n imum o f one (1) year Nur s e Pr a ctit i oner exper i ence a nd a min i mum of one ( 1) yea r hospi c e or p alliative car e ex p erience.
  • Current nursing licensur e i n State and CP R ce r tificat i on.
  • Accreditat i on f r om an app r oved ce r tifying b ody for a d vanced pra c tice nursing as required by State .
  • Excelle n t obse r vat i on, ver b al and writ t en communic a tion skill, p r obl e m so l ving skills, math e m a tical s kills; nursi n g skills per competen c y checklist.
  • Pro l onged or considerabl e walking o r standing. Able to lift , posit i on and / or tra n sfer patie n ts. Able to lift supplies and e q uipment. C onsiderabl e re a ching, stooping, bending, kneeling and/or crouching. Visual acuity a n d hearing to per f orm requ i red nursing skills.
  • Must be a li c ensed driver with an automobile tha t is i nsured in accordance with state/o r organization r equir e ment s and is in good working or d er.

Responsibilities

  • Provides medical ca r e, pall i ates, and manages te r mina l illness in compliance with the st a te Nur s e Practi c e A c t under t h e su p ervis i on of a c olla b orating physician.
  • Completes his t ory and physical ex a minat i ons, b y eva l uating the patie n t's medic a l condit i on and health history, ordering a n d interpreting r esults f r om labora t ory and diagnostic tests or procedure s , diagnosing h e alth condit i ons and d oc uments th e findings in the p a tient's medical record .
  • Manages h ealth condition s and te r minal illnesses by d eve l oping an individualize d plan of care, prescribing medicat i ons a n d / or t r eatment s , obtaining consultat i ons or making r eferrals and coordinating hospi c e ca r e s ervi c es.
  • Completes the hospi c e fa c e to face visit requirement s prior to the third and subsequent hosp i ce benefit per i ods to assess h ospice eligibility ba s ed on the hospi c e c ondition s of p a rticipation and the local coverage d ete r m i nations. Provides clinical i n formatio n to the hospi c e medical director for interpretat i on for eligibility for hospice r ece r tificat i on.
  • Makes recommendat i ons to the hospi c e medica l director r egarding patient eligibility for certific a tion and r ece r tific a tion for h ospice care.
  • Rev i ews clinical info r m a tion, coordinate s and ove r sees patient care in consultat i on wit h the hospice interdisciplinary team and othe r healthca r e p r actitio n ers.
  • Assures tha t patien t re c eives appropriate me a sures to cont r ol s ymptoms throu g h colla b orat i on with interdi s ciplinary te a m members.
  • Rev i ews and develop s proto c ols f or t r eatmen t and propo s es opt i ons fo r inte r ventions base d on the lite r ature in colla b orat i on wit h the colla b orating p hysician.
  • Consults the collabo r ating physician when the pat i en t 's plan of care i s outsi d e standardized practice and p r oto c ols.
  • Communic a tes and colla b orates with t h e interdi s ciplinary group to cre a te, r ev i ew and revi s e t h e patient's plan of ca r e.
  • Consults with t h e patie n t's attending physician, the h ospice medical di r ector and other healthcare pr a ctit i oners r e garding the patient's eligibility for hospice care, ongo i ng care n e eds and medica l mana g ement related to the pat i ent's te r minal c ondition.
  • Communic a tes with othe r communi t y hea l th pract i tioners to coordin a te t h e pla n of care.
  • Attends and part i cipates i n hospice interdisciplinary g r oup meetings .
  • Completes, maintains, and submits accurate and re l evant clinical notes , physica l ex a mination notes , asses s ment visit notes, medical orde r s, collaboration n ote s and other d o c umentat i on in the medical r ecord.
  • Educates/ c ounsels pat i ent s , famil i es, and/or careg i ve r s as to pr e ve n tative ca r e, medica l problems, p s ycho l ogical p r obl e ms, and spiritua l prob l em s in c onjunction with t h e interdisciplinary te a m to meet the tota l need s o f pati e nts.
  • Provides and mai n tains a s afe e n viro n men t fo r the p a tient.
  • Assists t h e patie n t and f a mily/ca r egiver and other te a m member s in providing continuity o f care .
  • Wor k s in c ooperat i on w i th the family/careg i ve r and hospice i n terdisciplinary group to identi f y the goal s o f car e and meet the ca r e need s o f the p a tient and fami l y/caregiver.
  • Serve s as a nursing re s our c e fo r consultat i on and edu c ations to mem b ers of t h e i nterdisciplinary te a m and other h ealthcar e practitio n ers in t h e c omm u nity.
  • Establishes, builds and nur t ures relat i onships with sta f f and community referra l sources to facilitate progr a m g r owth.
  • Maintains k n ow l edge of a n d compliance with cur r ent Medica r e/Medicaid, state/federal rules and regulations for h ospice serv i ces
  • Ensures c ompliance with the Medicare condit i ons of participation and ot h er state regulat i ons govern the p r ovision of h ealthcare.
  • Complies with all Health I n surance Por t ability and A ccountability Act (H I PAA) r e q uireme n ts in accordan c e w i th fede r al, state and organizational p ol i cies.
  • Participa t es in organizat i onal monitoring of the quality of medi c al se r vic e s and q u ality improveme n t initiat i ves.
  • Assumes res p onsibility for personal g r owth. Deve l ops , maintain s and upgrades p r ofess i onal know l edge and pra c tice sk i lls through attendance at seminars , conferen c es and participation in continuing educat i on and i n-servi c e clas s es.
