About The Position

The Inpatient Center Personal Care Worker (PCW) provides non-medical and supportive services to assist in palliative and end of life care to patients, families and caregivers in accordance with Rainbow Community Care (RCC) standards, practices and policies, within the scope of the PCW role and under the supervision of the RN.

Requirements

  • Valid Wisconsin driver’s license and proof of current car insurance is required upon hire and must be maintained throughout employment.
  • Must have a desire to work with individuals facing end-of-life issues.
  • High School diploma, GED, or equivalent combination of education and work experience.
  • Affirms the knowledge, philosophy and principles of hospice care.
  • Maintains patient confidentiality.
  • Must be able to maintain healthy boundaries and positive interpersonal relationships.
  • Must be tactful and courteous in contact and communication with others.
  • Presents self in a professional manner at all times.
  • Ability to act with integrity and demonstrate good work ethic including reliable attendance and punctuality.
  • Drives to meetings at other locations as necessary.
  • Required to work reasonable amounts of overtime (when approved by supervisor) and flexible hours.

Nice To Haves

  • Minimum of 6 months previous caregiving, homemaking or personal care worker experience preferred.

Responsibilities

  • Provides assistance with activities of daily living (ADLs), including tasks like bathing, dressing, eating, and toileting.
  • Assists with mobility and transfers as delegated and as directed by RN.
  • Provides housekeeping and laundry assistance as needed. Helps ensure patient environment is clean and organized.
  • Provides socialization and companionship to patients.
  • Assists with meal preparations.
  • Ensuring patients are comfortable, addressing their needs, and providing emotional support to the patient/family.
  • Reports concerns or observations about patient/family to a member of the patient’s core care team and/or to the one coordinating homemaker services.
  • Consistently and accurately documents activities as required by RCC protocols and standards.
  • Practices infection control techniques.
  • Other duties and tasks intended to maintain a safe and comfortable environment for patients and families.
