The ambulatory care pharmacist works collaboratively with providers, patients, and caregivers to provide patient‑centered care. Core responsibilities include monitoring medication levels and managing therapy to ensure medications are used safely and effectively. This involves ongoing clinical assessment, adjusting therapy based on laboratory results, supporting procedural planning, and providing comprehensive education to patients and families. Ambulatory care pharmacists provide guidance with challenges or barriers regarding medication management by connecting them with resources to support receiving highest level of care. Role will build strong partnerships with physicians, provide training to other staff members, and supports practice improvements. Through education, proper medication use, and monitoring, this will ensure therapies remain safe, consistent, and aligned with patient goals.
