About The Position

The he postgraduate year two (PGY-2) pharmacy residency at University of Louisville Hospital is accredited by the American Society of Health-System Pharmacists (ASHP) and offers a complete experience in multiple specialty areas. The PGY-2 program is a 12-month training program that begins in early July and ends June 30th of the following year. The completion of the program and receiving a certificate is dependent on successful completion of all competencies defined for the program. The program is structured according to ASHP’s Residency Program Design and Conduct and is designed to be flexible and individualized to the resident’s professional goals and interests. PGY-2 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and PGY-1 pharmacy residency programs to contribute to the development of clinical pharmacists in advanced or specialized practice. PGY-2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care that improves medication therapy. Residents who successfully complete an accredited PGY-2 pharmacy residency should possess competencies that qualify them for clinical pharmacist and/or faculty positions and position them to be eligible for attainment of board certification in that practice area (when board certification for the practice area exists). The fundamental goals of PGY-2 residency programs are to develop clinical practitioners who are capable of: Delivering pharmaceutical care to patients in several specialized areas with a focus on providing clinical pharmacy services in a proficient and compassionate manner Educating patients, students, and health care providers Functioning as an effective member of the health care team Demonstrating professional leadership, confidence, and effective communication skills Improving individual practice by self-evaluating one’s skills in providing pharmaceutical care Major job responsibilities include: medication order and/or outpatient prescription evaluation (to include but not limited to appropriateness and effectiveness) and verification, drug distribution support, technician supervision, clinical intervention, formulary management, patient education, and resource for nursing and physician medication related inquiries. The PGY-2 Pharmacy Resident will participate in the design, implementation, and monitoring associated with the provision of safe, effective and economical pharmacotherapy for all patients (inpatient and/or outpatient clinics). They will collaborate with preceptors and pharmacy leadership in the development and implementation of clinical pharmacy services and ensure compliance with policies and procedures at UofL Hospital.

Requirements

  • Graduated from an ACPE accredited College of Pharmacy (required)
  • Completion of Post-Graduate Year 1 (PGY-1) Pharmacy Residency (required)
  • Licensed or license-eligible as a pharmacist in the Commonwealth of Kentucky (required)
  • Current certification in Basic Life Support or must obtain within 90 days from date of hire (required)
  • Current Basic Life Support and Advanced Cardiac Life Support certification or must obtain within 90 days from date of hire (except, if not required of the PGY-2 curriculum)

Responsibilities

  • Delivers comprehensive pharmaceutical care to patients
  • Works collaboratively with other clinicians and health-care providers through multidisciplinary patient care rounds
  • Reviews medication orders for accuracy, appropriateness and efficacy
  • Documents all pharmacy interventions in patient electronic medical record and integrated clinical decision support system
  • Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
  • Provides pharmacokinetic consultations and other dosing recommendations, as required
  • Prevents, detects, monitors, documents, and reports adverse drug reactions and medications errors
  • Participates in the drug therapy management of medical emergencies (code blue, code stroke, etc.)
  • Performs both admission history and discharge medication review and/or counseling in order to facilitate the admission/discharge/transfer of patients
  • Provides operational support to Clinical Staff Pharmacists as determined by departmental need
  • Responds to drug information requests and optimize pharmacotherapy regimens utilizing current evidence-based literature
  • Promotes and demonstrates rational, cost-effective drug therapy
  • Participates in formulary management by implementing therapeutic interchanges and enforcing the non-formulary ordering process
  • Assists in the procurement of medication when necessary, including verification of home medication supply, when applicable
  • Facilitates the implementation of clinical and operational initiatives
  • Documents clinical consultations and cost avoidance as part of the departmental clinical documentation program
  • Prepares formulary review and drug use evaluations of medications for presentation to the Pharmacy and Therapeutics Committee
  • Assists pharmacy leadership in the determination, communication, and education of alternative therapies during medication shortages
  • Facilitates and participates in scholarly and professional development activities
  • Provides guidance and supervision to certified pharmacy technicians
  • Precepts pharmacy students during IPPE and APPE rotations as necessary
  • Participates in staff development (preceptor development/continuing education)
  • Participates in departmental research efforts
  • Actively participates in local, regional, or national organizations
  • Collaborates with pharmacy leadership in developing education, training, and competency programs
  • Design and deliver didactic lectures to Colleges of Pharmacy, Nursing and/or Medicine
  • Executes hospital and pharmacy policies and procedures
  • Collaborates with pharmacy leadership in development and review of departmental policies and procedures
  • Assists with recruitment and interviewing of new pharmacy residents
  • Completes sterile compounding training, including hazardous medications, and demonstrate competency on a frequency defined by the Sterile Compounding Supervisor
  • Completes extemporaneous compounding training, including hazardous medications, and demonstrate competency on a frequency defined by pharmacy leadership
  • Performs quality improvement projects as defined by pharmacy leadership
  • Investigates and resolves automated dispensing cabinet related issues
  • Covers assigned shifts as determined by departmental need
  • Facilitates investigational drug studies with assistance of Investigation Drug Service Manager and/or other personnel involved in the drug study
  • Utilizes Effective Communication Skills
  • Utilizes advanced communication skills to convey and demonstrate clinical pharmacist expertise
  • Forms collaborative relationships with pharmacy team members and other members of the healthcare team
  • Demonstrates effective professional and patient communications including articulation of recommendations
  • Demonstrates the ability to work in team environment, reaffirming the contribution of each person to the delivery of care/services
  • Maintains involvement in pharmacy department or hospital committees
  • Achieves and maintains satisfactory working relationships with pharmacy leadership, pharmacy staff and other departments throughout the system, including committees and administration
