About The Position

The pharmacist ensures prompt, accurate pharmaceutical care for patients, nurses, and providers. You are part of the Interdisciplinary Team that provides integrated health and healing activities for our community.

Requirements

• BPharm or PharmD.
• Licensed as a pharmacist in the State of Washington, or eligible for licensure within 2 months of hire.
• Basic computer skills.

Nice To Haves

• Previous hospital pharmacy experience.
• Pharmacy Residency and/or Board Certification optional.

Responsibilities

• Read, interpret and extract patient information from patient charts accurately.
• Monitors drug therapy regimens and adjusts per individual patient characteristics and needs.
• Review and verify medication orders before dispensing to patient.
• Dispenses medications to all areas of the hospital.
• Supplies drug information.
• Counsels and educates patients, patient family members, staff and providers on appropriate drug use.
• Doses drugs pharmacokinetically, monitors results, and adjust as indicated.
• Completes drug reviews.
• Supervises support personnel and assumes responsibility for final check of all completed work.
• Documents clinical activities and interventions accurately and completely.
