About The Position

Established in 1980, the Greater Lawrence Family Health Center (GLFHC) is a multi-site mission-driven non-profit organization employing over 700 staff whose primary focus is providing the highest quality patient care to residents throughout the Merrimack Valley. Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites throughout the service area and is the sponsoring organization for the Lawrence Family Medicine Residency program. GLFHC is currently seeking a Pharmacist to join our team for the Program of All-Inclusive Care for the Elderly (PACE) program. The PACE Program offers a unique opportunity to help our elderly population to continue to lead connected, meaningful lives. Under the direction of the Pharmacy Manager and the Adherence Packaging Lead Pharmacist, the PACE Pharmacy Technician supports the pharmacists with the prescription fulfillment process for all patients of the pharmacy. As the Adherence Packaging Lead, the Pharmacist is responsible for a wide range of medication management services and processes provided through the adherence packaging (i.e., Pill Strip Packaging with Parata Pass) and/or the P rogram of A ll-Inclusive C are for the E lderly (PACE). As part of the PACE interdisciplinary team, this pharmacist must be able to make evidence-based recommendations for optimizing drug therapy for our participants and will be responsible for PACE Enrollee Comprehensive Medication Review and Transitions of Care Medication Reconciliation. Must be able to don appropriate personal protective equipment (PPE).

Requirements

  • One to three years of experience.
  • Valid Driver’s License
  • Current BLS certification
  • Registration with State of Massachusetts as a Pharmacist

Responsibilities

  • Reviews and approves prescriptions to be filled by pharmacy technicians according to State Board of Pharmacy regulations.
  • Works with pharmacy technicians to perform the ‘final check’ of filled prescriptions to ensure accuracy and timely delivery.
  • Works with patient medical information to conduct Drug Utilization Review on all prescriptions and reviews clinical edits to ensure there is no adverse drug reaction.
  • Works in reconciling Controlled Substances Logs with inventory on hand and prescriptions dispensed to comply with state and federal regulations.
  • Provides clinical expertise by responding to drug-related questions.
  • Familiarity with order processing, inventory, Database software, prescription verification, billing and adjudication software.
  • Safely administer and accurately document immunizations and medications.
