The Pharmacy Care Coordinator – Clinical Support is a call center position primarily responsible for supporting the Care Center and Clinical Services staff in a large portion of medication management services and clinical programs, including telephonic patient outreach, project coordination, scheduling call back appointments, coordination of care, and follow up care activities. Hours: Monday – Friday – 10:30 AM – 7 PM CST This is a telecommuter position. You’ll enjoy the flexibility to work remotely from anywhere within the U.S. as you take on some tough challenges.

Requirements

  • High School Diploma/GED (or higher)
  • 6+ months of experience working with healthcare operations (i.e. Clinic, Pharmacy, Scribe, etc.) PBM, and/or Health Plan
  • All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.

Responsibilities

  • Supports the Clinical Services staff with daily patient outreach calls, call back appointment scheduling, contacting providers and pharmacies and other patient care activities as appropriate
  • Completes support tasks to increase the efficiency of interactions between patients, pharmacists, and other clinical staff to maximize the number of patient encounters and bridge gaps in communication
  • Coordinates care between patients and clinical staff to optimize access to Genoa’s clinical services and pharmacy site services
  • Identifies and resolves pharmacy care needs related to access, synchronization, affordability, and adherence packaging
  • Provides follow-up support services to evaluate and ensure a positive patient experience
  • Maintains department productivity levels for patient satisfaction and performance goals
  • Other tasks as assigned
