About The Position

The Pharmacy Population Health Coordinator supports clinical pharmacy programs within the Beth Israel Lahey Health Performance Network (BILHPN) – Clinical Excellence Division. The role focuses on improving medication adherence, closing care gaps, and enabling performance in our value‑based payer contracts. Working under the direction of Clinical Pharmacy Specialists and pharmacy leadership, the coordinator carries out program operations, supports population‑health‑related pharmacy activities, and collaborates with interdisciplinary teams across the network.

Requirements

  • High school graduate with college degree preferred.
  • Certified Pharmacy Technician in Massachusetts; willing to obtain additional state licensure as needed.
  • 2 to 5 years of experience in retail pharmacy, managed care, or population health environment.
  • Strong communication, time management, and organizational skills.
  • Ability to work independently within scope of work.
  • Ability to communicate effectively with healthcare professionals and patients in person and telephonically.
  • Ability to research and analyze medical literature, drug information, evidence-based guidelines and pharmacy, and cost utilization data.
  • MS Office applications including Word, PowerPoint, Excel and Outlook required; Experience with databases preferred.
  • Experience with Epic Electronic Medical Record preferred.

Nice To Haves

  • Experience with medication adherence, value‑based care contracts, and motivational interviewing skills preferred.
  • Knowledge of HEDIS, Stars Quality metrics preferred.
  • Knowledge of Massachusetts / national Prescription Assistance Programs.

Responsibilities

  • Review medication adherence, quality, and performance reports to identify patients needing intervention.
  • Conduct outreach to patients, providers, pharmacies, and care teams to address adherence gaps, medication access issues, and pharmacy benefit questions.
  • Assist with addressing barriers to medication access including coordination of refills and prescription assistance programs.
  • Route prioritized intervention opportunities to clinical pharmacists based on workflow and risk criteria.
  • Support pharmacy-led population health and quality improvement initiatives.
  • Perform administrative and operational tasks allowing clinical pharmacists to practice at top-of-license.
  • Document all activities in the electronic medical record per established workflow.
  • Participate in program data tracking, reporting, workflow optimization, and resource development.
  • Participate in the education of PGY1 and PGY2 pharmacy residents and pharmacy students where applicable.
  • Maintain adherence to confidentiality policy
  • Comply with BILH/BILHPN policies
  • Maintain required certifications and competencies
  • Maintain courteous and effective interactions with colleagues and patients.
  • Participate in all department meetings
