Requirements

  • Graduate of an accredited Physical Therapist Assistant (PTA) program
  • Active PTA license in the state of practice
  • Ability to work independently while supporting a multidisciplinary healthcare team
  • Ability to perform essential job functions with or without reasonable accommodation

Nice To Haves

  • Experience in SNF, Assisted Living, long-term care, or post-acute rehabilitation preferred
  • Strong communication and clinical collaboration skills

Responsibilities

  • Provide physical therapy treatments in Skilled Nursing Facilities (SNF) and Assisted Living (ALF) settings under the direction of a licensed Physical Therapist
  • Deliver evidence-based geriatric rehabilitation and post-acute care services
  • Implement individualized treatment plans focused on mobility, strength, balance, and functional independence
  • Document daily treatments and patient progress in compliance with Medicare, PDPM, and state Physical Therapy regulations
  • Participate in patient care conferences and interdisciplinary rehabilitation meetings
  • Communicate effectively with supervising Physical Therapists, Rehab Directors, nursing staff, and care teams
  • Support quality outcomes in long-term care and post-acute rehabilitation environments
