Requirements

  • Current, valid Physical Therapist Assistant (PTA) License in the state of California.
  • A positive, team-oriented attitude and a passion for geriatric rehabilitation.
  • Strong communication skills and the ability to build meaningful relationships with residents and families.

Responsibilities

  • Treatment Delivery: Provide skilled physical therapy interventions as directed by the supervising Physical Therapist to help residents regain mobility and independence.
  • Clinical Documentation: Maintain accurate daily treatment notes and weekly progress reports in accordance with PT Board standards.
  • Interdisciplinary Collaboration: Work closely with nursing staff and assist in training Restorative Aides to ensure continuity of care.
  • Patient & Family Education: Instruct residents and caregivers on maintenance programs and home exercise routines to facilitate safe discharge planning.
  • Team Support: Participate in care conferences and communicate effectively with the healthcare team regarding patient progress and care plans.
  • Environment Safety: Ensure therapy areas are clean, safe, and well-maintained for our residents.
