Requirements
- Graduate of an APTA-approved Physical Therapy program
- Active, unrestricted Physical Therapist license in the state of practice
- At least one year of experience in a healthcare setting
- Willingness to travel within the assigned coverage area (up to 50%)
- Reliable transportation, valid driver’s license, and current auto insurance
- Ability to lift 50–100 lbs and perform essential physical functions of the role
Responsibilities
- Deliver hands-on, patient-centered care in the comfort of patients’ homes
- Evaluate functional needs and create personalized plans of care
- Track progress, adjust treatment plans, and celebrate patient wins
- Collaborate with physicians, caregivers, and interdisciplinary teams
- Mentor and guide therapy assistants and support staff
- Complete timely, accurate documentation to support excellent outcomes
- Act as a clinical ambassador for physical therapy in the community
