About The Position

The Physical Therapist (PT) – Acute Care Hospital evaluates, plans, and implements individualized rehabilitation programs for patients recovering from injury, illness, or surgery in an acute care hospital setting. The therapist collaborates with physicians and multidisciplinary healthcare teams to improve patient mobility, functional independence, and overall recovery outcomes. This role requires strong clinical assessment skills, the ability to develop patient-specific treatment goals, and experience providing rehabilitation services in a fast-paced hospital environment.

Requirements

  • Bachelor’s Degree in Physical Therapy .
  • New York State Physical Therapist License or eligibility to obtain licensure.
  • Minimum 2 years of physical therapy experience , preferably in an acute care hospital setting.
  • Basic Life Support (BLS) certification preferred.
  • Ability to pass pre-employment health clearance .

Responsibilities

  • Evaluate patients’ physical conditions and functional limitations to develop individualized treatment plans.
  • Design and implement rehabilitation programs based on patient needs and physician orders.
  • Establish specific treatment goals in collaboration with patients and the interdisciplinary healthcare team.
  • Provide therapeutic interventions including mobility training, strengthening exercises, and functional rehabilitation .
  • Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
  • Maintain accurate and timely clinical documentation, treatment notes, and discharge summaries .
  • Participate in patient rounds, clinical conferences, and interdisciplinary care discussions .
  • Provide patient and family education regarding home exercise programs and recovery strategies .
  • Assist in training new staff members and supervising rehabilitation students when applicable.
  • Contribute to program development and quality improvement initiatives within the rehabilitation department.
