Physical Therapist (PT) – Home Health | Full-Time, Part-Time or PRN King George, VA & Massaponax, VA Competitive Pay | Flexible Scheduling | Mileage Reimbursement Human Touch Home Health is seeking an experienced Physical Therapist (PT) to provide skilled in-home therapy services in King George and Massaponax, Virginia . We are currently hiring for Full-Time, Part-Time, or PRN positions based on your availability and preference. Requirements

• Active Virginia Physical Therapist license
• Minimum 1–2 years PT experience (home health preferred)
• OASIS experience preferred
• EMR experience (WellSky/Kinnser or Homecare Homebase a plus)
• Strong clinical and documentation skills
• Valid driver’s license & reliable transportation

Responsibilities

• Perform comprehensive in-home PT evaluations and reassessments
• Develop and implement individualized treatment plans per physician orders
• Provide skilled therapy to improve mobility, strength, balance, and function
• Complete accurate and timely OASIS documentation
• Educate patients and caregivers on home exercise programs and safety
• Collaborate with physicians and interdisciplinary team members
• Supervise Physical Therapist Assistants (PTAs) as applicable
