Physical Therapist (PT) – PRN | Home Health Blacksburg, VA & Surrounding Areas Human Touch Home Health is seeking a licensed Physical Therapist (PT) for a PRN (as-needed) home health position serving patients in Blacksburg and surrounding communities. This role is ideal for clinicians seeking flexibility and autonomy while providing meaningful, one-on-one care in the home setting. Position Overview The Physical Therapist will provide skilled therapy services in the home setting, supporting patients in improving mobility, strength, balance, and overall function. This position requires independent clinical judgment and coordination with the care team.
Requirements
- Active Virginia Physical Therapist license
- Minimum 1 year of experience (home health preferred)
- Strong clinical and documentation skills
- Valid driver’s license and reliable transportation
Nice To Haves
- OASIS experience preferred
- EMR experience preferred
Responsibilities
- Perform in-home PT evaluations and reassessments
- Develop and implement individualized treatment plans
- Provide therapeutic interventions per physician orders
- Complete documentation accurately and timely
- Educate patients and caregivers on home exercise programs
- Collaborate with physicians and interdisciplinary team members
- Supervise PTAs as applicable
