Physical Therapist (PT) & Physical Therapist Assistant (PTA) — Per Diem Pay: PT: $80 per visit PTA: $55–$60 per visit Location: O’ahu, HI (reliable transportation required) Schedule: Flexible / Per diem — choose your patients About Us We’re a growing Medicare-certified Home Health agency on O’ahu focused on fall prevention, mobility, and helping seniors stay safe and independent at home. Requirements

• Current Hawaii license as PT or PTA
• 1+ year clinical experience (home health preferred)
• Strong documentation and communication skills
• Driver’s license + reliable transportation

Responsibilities

• Provide in-home physical therapy visits
• Complete evaluations, reassessments, and fall risk assessments
• Document accurately and communicate progress with the care team
