The Position : Physical Therapist Employment Status : Full-time Hours per two-week pay-period: 70-80 hours Position Summary: Sidney Health Center is seeking a Physical Therapist to join our I CARE team in the Rehab setting. Essential duties of the position include performing comprehensive evaluations and establishing individualized plans of care, providing skilled physical therapy interventions, and document evaluations, treatments, progress notes, and discharge summaries accurately and timely in EMR. Requirements

  • Graduate of an accredited Physical Therapy program (DPT or Masters in Physical Therapy)
  • Current Physical Therapist license in MT or eligibility to obtain
  • Experience in hospital-based, CAH, or rural health settings
  • Current MT Physical Therapist license
  • CPR certification

Responsibilities

  • Evaluate patients and develop individualized plans
  • Provide skilled physical therapy interventions across appropriate settings (outpatient, inpatient, swing bed, or SNF)
  • Monitor patient progress, modify treatment plans, and document outcomes in the electronic medial record
