About The Position

Astrana Health is seeking a skilled and compassionate Primary Care NP/PA to join our growing outpatient clinic in Anaheim, CA with Gateway Medical Center. This role is ideal for a provider who is passionate about delivering high-quality, patient-centered care with a strong focus on preventive medicine, chronic disease management, and value-based care. Gateway Medical Center is a multi-specialty medical practice that has served the Anaheim area since 1984. We provide comprehensive primary medical care to our patients out of our three clinic locations found in Anaheim, Anaheim Hills and Santa Ana California. Spanish-speaking providers and those with geriatric experience are strongly preferred. Requirements

  • Active and unrestricted NP/PA license in CA
  • Fluent Spanish
  • Active DEA registration
  • Current BLS and ACLS certifications
  • Minimum 1–2 years of outpatient primary care experience
  • (New graduates will be considered)

  • Geriatric care experience
  • Experience in value-based care, community health, or managed care environments
  • Strong communication, teamwork, and clinical judgment skills
  • Passion for serving diverse and underserved populations

Responsibilities

  • Provide and manage comprehensive primary care services, including:
  • Physical examinations, evaluations, assessments, diagnoses, counseling, and treatment
  • Preventive care visits, wellness exams, and age-appropriate screenings
  • Manage chronic conditions such as diabetes, hypertension, COPD, heart disease, and other common adult medical conditions
  • Prescribe medications and treatment regimens in accordance with clinical guidelines and best practices
  • Develop individualized, evidence-based care plans in collaboration with patients and families
  • Maintain a continuity panel of patients under supervising physician with appropriate follow-up and long-term care planning
  • Collect, review, and maintain complete and accurate patient medical records in the EHR
  • Order, interpret, and follow up on diagnostic tests, labs, and imaging
  • Coordinate care with specialists, behavioral health providers, and ancillary services
  • Direct and collaborate with nursing and support staff to ensure efficient patient flow and high-quality care
  • Participate in quality improvement initiatives, including HCC, 5-Star, and other incentive-based programs
  • Advocate for patients within the healthcare system to support cost-effective, coordinated care
  • Recommend workflow and operational improvements to enhance clinical efficiency, documentation, and patient experience
  • Uphold the highest standards of medical ethics, professionalism, and patient confidentiality
  • Other duties as assigned.
