About the position

Theoria Medical is seeking a part-time physician to serve as the attending physician for residents in skilled nursing facilities (SNFs). This role involves collaborating with Nurse Practitioners and Physician Assistants to manage day-to-day resident care, providing high-quality, patient-centered primary care services, and participating in various medical quality initiatives. The position offers flexible scheduling with no overnight duties, weekends, or on-call requirements, allowing for a balanced work-life experience. Physicians will utilize electronic health record systems for documentation and will be supported by a virtual care team.

Responsibilities

  • Serve as the attending physician for residents at each facility.
  • Collaborate with Nurse Practitioners and Physician Assistants for daily resident care.
  • Provide comprehensive primary care services by rounding on patients in SNFs.
  • Diagnose and treat patients with acute illnesses and chronic diseases.
  • Complete provider documentation in Theoria’s proprietary EHR system, ChartEasy™.
  • Participate in Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives.
  • Supervise collaborating nurse practitioners and physician assistants.
  • Utilize electronic/online medical tools for administrative functions.

Requirements

  • Medical degree and valid medical license.
  • Experience in primary care or related field.
  • Ability to collaborate effectively with a healthcare team.
  • Proficiency in using electronic health record systems.

Nice-to-haves

  • Experience in skilled nursing facilities.
  • Familiarity with PointClickCare, MatrixCare, and American HealthTech EHR systems.
