About The Position

Community Health Alliance is looking for a Population Health Manager. We operate six convenient locations throughout Reno and Sparks, providing comprehensive, top-quality medical services, dental and pediatric care, behavioral health, low-cost pharmacies, nourishing food pantries, and more. Job Summary: The Population Health Manager oversees value-based agreement projects, including outreach initiatives, quality metric tracking, and value-based reporting. This role manages the Community Health Worker (CHW) program and the specialty network referral processes through the Care Coordination team. The Population Health Manager supports the development and implementation of value-based programs that emphasize prevention and early intervention to improve health outcomes and increase access to health care and community resources Our team members enjoy benefits that include: Competitive Salary: $68,000-$74,000 100% company paid employee medical insurance and 90% paid vision and dental on the 60th day from date of hire. Requirements

  • Ability to implement and manage action plans aligned with value-based contract goals and performance metrics, including patient outreach requirements.
  • Strong understanding of value-based agreements, incentive structures, and performance reporting.
  • Knowledge of quality improvement methodologies and population health best practices.
  • Working knowledge of Medicaid, Medicare, and commercial health plan programs.
  • Proficiency in electronic medical record (EMR) systems and healthcare data platforms.
  • Demonstrated leadership and management skills, including performance evaluation, effective communication, time management, and conflict resolution.
  • Experience managing multiple complex initiatives across organizational platforms in collaboration with leadership.
  • Knowledge of health clinic operations, including front office workflows, practices, and procedures.
  • Proficiency with Microsoft Excel, PowerPoint, Microsoft Office Suite, and project management tools.
  • Ability to navigate provider and health plan portals and retrieve relevant information efficiently.
  • Strong analytical skills, including report development and the ability to prioritize projects using data-driven insights.
  • Knowledge of medical billing and coding practices, CPT/ICD codes, and medical terminology.
  • High level of accuracy, attention to detail, organization, and project timeline management skills.
  • Ability to maintain confidentiality of medical and financial information in compliance with HIPAA regulations.
  • Knowledge of HRSA requirements, NCQA standards, and HEDIS quality measures.
  • Ability to exercise sound judgment and make informed decisions in complex situations.
  • Ability to work collaboratively and build positive relationships across multidisciplinary teams and throughout the organization.
  • Exceptional verbal and written communication skills and a high level of professionalism.
  • Ability to work effectively and sensitively with diverse populations.
  • Bachelor’s degree in Health Sciences, Public Health, Social Work, Human Services, or a related field required.
  • Minimum of two (2) years of experience in the healthcare services field, including knowledge of community resources, social drivers of health, and familiarity with local specialty network providers required.
  • Minimum of two (2) years of experience managing a healthcare team of at least three (3) staff members required.
  • Ability to maintain a valid driver’s license and travel between clinic sites as required.

  • Ability to lift up to 20 pounds on an occasional basis.
  • Ability to sit, stand, walk, and perform repetitive tasks associated with office and clinical environments.
