We are seeking a strategic and collaborative Population Health Program Manager to lead initiatives that improve outcomes, enhance care coordination, and advance high-performance population health operations. Position Purpose: The Population Health Program Manager is responsible for the successful planning and execution of key strategic activities that will enable the rapid advancement of high-performance population health operations. This position will partner closely with operations, analytics, and clinical leadership, including all areas of population health operations. The Population Health Program Manager will develop tactics to meet program goals and metrics and facilitate key leadership meetings to collaborate cross-departmentally communicating and holding teams accountable to progress against program objectives and goals. This is a full-time, FLSA Exempt position.

Requirements

  • Master's degree in Nursing or closely related field
  • Current, current AZ RN License
  • 5 years of population health, quality/process improvement, nursing leadership, or project management
  • Excellent communication and presentation skills. Effective in working with all levels of clinicians (physicians, mid-levels, nurses, allied, office staff, etc.) Ability to lead through influence by establishing and maintaining working relationships with peers and stakeholders at all levels of the organization. Demonstrated project management and organizational skills. Robust analytics and problem-solving skills that allow for thoughtful assessment of decisions and associated implications. Illustrated knowledge of both healthcare operations as well as health insurance/health plans.

Responsibilities

  • Strategic Leadership & Program Execution: Support strategic planning and execution of the population health work plan aligned with organizational priorities.Monitor key performance indicators and program outcomes to ensure best-in-class performance. Partner with organizational stakeholders to develop and integrate population health initiatives
  • Collaboration & Leadership Engagement: Lead and develop content for key committees overseeing population health performance and initiatives. Facilitiate leadership meetings that promote cross-department collaboration and accountability. Serve as a liaison with external partners, including payers and community organizations. 
  • Data & Performance Improvement: Collaborate with analytics teams to develop reporting tools and actionable performance insights. Use data to evaluate outcomes and guide improvements.
  • Care Transformation & Provider Engagement: Act as a change agent to support care transformation and improved care delivery workflows. Build strong physician partnerships by creating shared vision and fostering trust. Align providers and teams to program goals and performance expectations. 
  • Program Coordination & Team Leadership: Partner with operations, analytics, and clinical leadership to ensure alignment and program success. Ensure teams understand deadlines, requirements, and responsibilities. Supervise nurse navigators in outpatient clinics.
