About The Position

The Population Health Specialist supports the planning, implementation, and coordination of population health initiatives across NEMS, including the various Medicare and Medi-Cal value-based care (VBC) programs' performance and improvement activities. This includes identifying and addressing care gaps, supporting patient outreach strategies, analyzing and validating population health data, facilitating program implementations, and collaborating with clinical and non-clinical teams to improve health outcomes for the NEMS patient populations. The Population Health Specialist will also work closely with contracted Health Plan(s) to ensure the success of VBC programs. Candidate in this position is required to be able to effectively communicate with a variety of professionals, including physician providers, health plan program administrators, and other NEMS internal departmental managers and staff, to promote effective program activities and achieve optimal program outcomes.

Requirements

  • Bachelor’s degree with minimum of 2 years of experience working in healthcare setting. Associate degree may be considered with relevant, equivalent work experience.
  • Strong Excel, Word, PowerPoint, and Outlook skills.
  • Must be able to fluently speak, read and write English.
  • Fluent in Chinese (Cantonese and/or Mandarin) required.
  • Detail oriented and strong critical thinking and problem-solving skills to manage multiple levels of information and priorities, to quickly assess issues as they arise, and to develop timely and effective solutions.
  • Strong interpersonal skills to collaborate effectively with a broad range of internal and external stakeholders.
  • Excellent communication (spoken and written) and time management skills.

Nice To Haves

  • Knowledge of managed care, Medi-Cal, Medicare, and HEDIS is strongly preferred.
  • Experience with patient outreach, care gap closure, or healthcare quality initiatives preferred.
  • Knowledge of QI standards, measurement tools and techniques including collection, aggregation, abstraction, validation, analysis, and reporting of data preferred.
  • Prior experience in a managed care setting or IPA provider organization a plus.
  • Fluency in other languages is an asset.

Responsibilities

  • Coordinate and support population health and value-based care (VBC) initiatives across NEMS to improve preventive care, chronic disease management, and care gap closure.
  • Conduct targeted patient outreach through phone, mail, and virtual platforms to promote preventive screenings, immunizations, and chronic disease follow-up.
  • Collect, analyze, and interpret population health and VBC data from medical records, registries, and databases to monitor performance against quality and HEDIS metrics.
  • Compile and review statistical data to support Medicare, Medi-Cal, and other VBC programs, including inter-rater reliability studies, provider utilization analyses, and quality improvement studies.
  • Collaborate with physicians, care teams, data analysts, and external partners to identify trends, barriers to care, and opportunities for improvement.
  • Utilize population health registries, reporting tools, and informatic systems to manage outreach activities, track interventions, and support program analytics.
  • Prepare data-driven reports, summaries, and visual presentations to communicate program outcomes and support decision-making by internal and external stakeholders.
  • Coordinate implementation of intervention strategies and sustainable workflows recommended by the NEMS Practice Improvement Program (PIP) Committee.
  • Monitor program performance and ensure timely submission of required reports and progress summaries to leadership and Health Plan(s).
  • Participate in Performance Improvement/Continuous Quality Improvement (PI/CQI) activities, committees, and preparation of related meeting materials.
  • Performs other job duties as required by manager/supervisor.
