Primary Care Physician

Transportation is provided to patients that need it at no charge.

Responsibilities

• Provide primary care to a patient panel of seniors with 3-5 chronic diseases.
• Manage a maximum of 450 patients.
• Schedule and conduct patient appointments during clinic hours from 7:30 AM to 5:00 PM, Monday to Friday.
• Participate in value-based care initiatives.
• Work collaboratively with a dedicated care and support team, including a Medical Assistant and Scheduler.
• Perform minor wound closures, joint injections, skin biopsies, and comprehensive wound care.

Requirements

• M.D or D.O.
• Clear and Active medical license.
• Active DEA license.
• Board certified in Internal Medicine, Family Medicine or Geriatric Medicine.
• 2+ years of experience in Primary Care Outpatient setting.
• Bilingual: English & Spanish (preferred, not required).
• Proficiency in suturing for minor wound closures.
• Competence in administering joint injections for pain management.
• Capability to perform skin biopsies for diagnostic purposes.
• Ability to manage and provide comprehensive wound care.
