The Role As a Primary Care Physician, you’ll serve as the central provider for your patient panel, delivering high-quality, comprehensive care in home-based and community settings , supported by an experienced interdisciplinary team.
Requirements
- MD or DO with an active California medical license
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Comfortable with EMR systems and team-based care models
- Compassionate, communicative, and patient-centered
Nice To Haves
- Experience with geriatric or senior-focused care (strongly preferred)
Responsibilities
- Providing preventive, acute, and chronic care for senior patients
- Conducting comprehensive health assessments and managing personalized care plans
- Ordering and interpreting labs and diagnostics
- Collaborating closely with Nurse Practitioners, Physician Assistants, and Wellness Coordinators
- Leading population health and quality improvement initiatives
- Mentoring and supporting advanced practice providers
- Participating in provider meetings and continuing clinical development
