About The Position

The Primary Care Physician will provide onsite clinical services to eligible beneficiaries of the Charlie Norwood VA Medical Center. You will be responsible for delivering high-quality primary care that meets or exceeds the standards set by the American College of Physicians (ACP) and the American Academy of Family Physicians (AAFP). $551,200 - $551,200 a year Location: Charlie Norwood VA Medical Center, One Freedom Way, Augusta, GA 30904. Schedule: 8:00a – 4:30p

Requirements

Must be Board Certified or Board Eligible (ABIM, ABFM, AOBIM, AOBFP).
Must hold a current, full, and unrestricted license to practice medicine in any U.S. State, Territory, Commonwealth, or the District of Columbia.
Must possess a National Provider Identifier (NPI) and a current DEA certificate.

Responsibilities

Provide direct primary care services, including diagnosis, treatment, and management of acute and chronic conditions for Veterans.
Preventive care of immunizations, cancer screening, and lifestyle counseling.
Chronic disease management of Diabetes, hypertension, COPD, heat failure, etc.
Acute care evaluations and treatment of new or urgent symptoms.
Mental Health screening for depression, PTSD, and substance use (with referrals)
Serve as the team leader in the Patient Aligned Care Team (PACT) model with coordination with specialists, mental health providers, Social Workers, Pharmacists, and Community care providers (via VA’s Community Care Network).
Manage referrals, consults, and follow-up care.
Educate veterans on nutrition, exercise, smoking cessation, and alcohol use.
Use clinical reminders and health maintenance tools in the electronic health record to track preventive care.
Address aging-related issues, including cognitive decline, falls, and polypharmacy.
Collaborate with Geriatrics and Extended Care (GEC) and Palliative Care teams when appropriate.
Provide gender-specific care.
Address reproductive health, menopause, and trauma-informed care.
Conduct video visits, telephone appointments, and secure messaging via My HealtheVet.
Use remote patient monitoring tools when available.
