Schedule: This position is 28 hours per week, with flexibility to work either three 10-hour days or four 7-hour days based on the candidate’s preference. The role requires participation in Interdisciplinary Team (IDT) meetings every other Thursday at 7:00 a.m. The territory includes Sturgeon Bay and north, covering all of northern Door County, including Washington Island. One holiday per year is required, along with on-call and holiday expectations as outlined below. On-call responsibilities include either one full weekend every other month or one weekend shift per month, whichever fits your preferred rotation. Weekend call shifts run Friday 5 p.m.–9 p.m., and Saturdays and Sundays 8 a.m.–5 p.m. or 11 a.m.–8 p.m. One holiday per year is required, with holiday shifts running 8 a.m.–5 p.m. or 11 a.m.–8 p.m. As a Social Worker, you will play an essential role on our interdisciplinary team, supporting patients and families through the end-of-life journey.
