About The Position
PRN Hospice Social Worker (MSW) Varco Hospice is seeking a compassionate and hospice-experienced Social Worker (MSW) to join our interdisciplinary team. This is a PRN position only supporting patients and families throughout the end-of-life journey.
Requirements
- Active Texas Social Work license (MSW required; BSW considered per hospice regulations)
- Minimum of 2 years hospice social work experience required
- One year of healthcare social work experience
- Current TB or chest X-ray and CPR certification
- Valid driver’s license and willingness to travel to patient homes and facilities
- Strong communication and documentation skills
Responsibilities
- Provide medical social services in accordance with the patient’s Plan of Care
- Complete psychosocial assessments within 5 days of hospice admission
- Support patients and families with end-of-life planning, coping, and advocacy
- Assist with community resources, advance planning, and discharge planning as needed
- Observe, document, and report changes in patient and family psychosocial status
- Participate in IDG and QAPI meetings
- Maintain accurate, timely clinical documentation
- Serve as a resource and consultant to the interdisciplinary team
