A Calling with Impact At Lifted Hospice, our Nurse Practitioners bring clinical expertise and spiritual presence into every interaction—helping patients live fully and comfortably, even in life’s final chapter. As a Nurse Practitioner, you serve as a trusted clinical leader and guide—supporting the hospice physician, collaborating with attending providers, and helping oversee the care of patients and families. You bring clarity to complex situations, lead with compassion, and provide expert symptom management that honors each person’s goals and dignity. Your Role in the Mission As a Nurse Practitioner at Lifted Hospice, you serve as a clinical leader within the interdisciplinary team—ensuring that every patient’s care reflects their goals, honors their dignity, and aligns with hospice standards. You partner closely with attending physicians and hospice physicians to support symptom management, guide eligibility decisions, and provide clinical oversight throughout the hospice journey. Your face-to-face visits, assessments, and expert communication help ensure appropriate, timely care. You also strengthen the team by offering consultation and education—helping nurses, social workers, chaplains, and others deliver unified, high-quality care. Through your leadership and presence, you help families feel supported, teams feel equipped, and patients feel seen and cared for at every stage. In this role, you’ll travel to see patients wherever they call home—whether that’s a private residence, assisted living, memory care, or skilled nursing facility. You’ll meet patients and families in person to complete face-to-face visits, assess clinical needs, and support care decisions with clarity and compassion. You’ll collaborate closely with your local interdisciplinary team—fellow nurses, social workers, chaplains, physicians, and our medical director—to align care plans and ensure every patient receives thoughtful, timely, and coordinated care. While your visits may vary in frequency, your presence plays a key role in the continuity, quality, and clinical integrity of our care model. This position requires local travel, flexibility in scheduling, and the ability to adapt to different care environments while maintaining professionalism, presence, and clinical excellence.
