SUMMARYCompiles, verifies, types, and files individual records. Responds to requests for medical records and performs other assigned clerical duties. Operates computer to enter and retrieve individual data. ESSENTIAL JOB FUNCTIONSTo perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below: Maintains files and individual records in a timely manner by updating and filing client data upon receipt of information. Initiates records for new clients and creates computer index. Prepares file labels and maintains and audits filing sequence. Assists in copying and distribution of record information per policy. Sends and receives information via facsimile machine. Operates computer to enter and retrieve individual data. Maintains forms supply, including packet preparation.Reviews clinical documentation prepared by program staff to ensure that timecard and clinical documentation coincide. Ensures accuracy of all data. Provides appropriate and timely follow-up and tracking as needed. Forwards appropriate documents and information to other program staff upon request.Keeps supervisor informed of problems or issues. Performs other related duties and activities as required. SUPERVISORY RESPONSIBILITIES None required. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Education and Experience: High School Diploma or equivalent and six months of general office experienceExperience in medical records preferred Certificates, Licenses, and Registrations: Valid driving license and registration for the state if providing transportation services.
