The Clinical Care Coordinator, under the direction of the program manager, is an active member of the patient treatment team.
Requirements
Graduate degree in social work, mental health counseling, marriage and family therapy, or related field.
A minimum of 1 year experience working with individuals with persistent mental illness (PMI) and/or substance abuse.
CPR certification and successful completion of restraining procedures training within 30 days of employment.
Responsibilities
Provide milieu management and continuous patient care supervision, interactions and role modeling to patients.
Provide support at the front desk, including:
Maintaining patient transportation schedule, including scheduling Lyfts as needed
Answering phones
Completing phone assessments with potential clients and adding them to Windmoor Call Log
Providing required paperwork for intakes, discharges, and weekly outcome measures
Entering outcome measures in Reflections tracking database
Supervising and updating daily attendance tracker
Tracking patient attendance, absences, and length of treatment
Following no-call/no-show protocol as needed to ensure patient safety
Meeting with potential stepdown patients from inpatient units for registration
Providing support to clinical staff by completing admission assessments, treatment plans, and/or facilitating group sessions.
Assisting patients with obtaining resources and referrals as needed.
Participating in additional tasks as needed through assignment by the program manager.
