Collaborate with Family Medicine Care Coordinators and interdisciplinary teams. Develop and implement personalized health care plans for patients. Facilitate effective communication between patients and health care providers. Monitor patient progress and adjust care plans as necessary. Provide education and resources to patients and their families. Document and maintain accurate patient records and data. Ensure compliance with healthcare regulations and standards. Requirements Bachelor’s degree in Health Education or related field, or California LVN license. Experience in a healthcare setting preferred. Strong written and verbal communication skills. Proficient in Microsoft Office and electronic health record systems. Ability to work independently and as part of a team. Excellent organizational skills and attention to detail.
