About The Position

The Advance Care Planning Program Manager provides expertise in daily operational functions of clinical programs within the scope of their licensure. This role ensures the advance care planning programs are operating effectively, through direct participation, reporting, oversight, and providing clinical education to other care team members. Extensive knowledge of patient care workflows, care delivery goals, and medical record documentation processes. Assists the Director of Advance Care Planning and Social Work with program development, education, and integration of Advance Care Planning in care pathways for all adult populations to improve performance and outcomes. This is a remote/work from home position. Knowledge of APC law in VA and/or SC is strongly preferred.

Requirements

  • Masters- According to specialty area (social work, nursing, behavioral health, other)
  • Appropriate to clinical/care specialty - CNS, LSW, LISW, LCSW, or RN (required)
  • 4 years of clinical practice and experience in care setting (required)
  • Excellent interpersonal communication and negotiation skills.
  • Strong analytical, data management and computer skills.
  • Knowledge of evidence based practice and interventions required to improve patient outcomes.

Nice To Haves

  • MSW or BSN- According to specialty area (nursing, social work)
  • ACP Instructor Certification (preferred)
  • Demonstrated success in improving the ACP outcomes of a distinct population of patients in ambulatory, inpatient or community setting.
  • Ability to influence and motivate teams to adopt processes.
  • Knowledge of APC law in VA and/or SC is strongly preferred.

Responsibilities

  • Coordinates daily operations within department, with functions related to healthcare ethics, palliative care and advance care planning (ACP).
  • Provides recommendations on the clinical pathways to improve reliable and effective ACP for Well, Rising Risk, and Seriously Ill patient populations.
  • Monitors and measures progress toward the achievement of departmental, programmatic, and system goals.
  • Establishes strong, consistent relationships with community organizations and key internal and external stakeholders; to provide leadership to pilot/implementation project site leaders and better acquaint the system with the programs offered through the department.
  • Implements system level Advance Care planning processes as they relate to the goals of Care Transformation, Population Health, and Serious Illness programs.
  • Using clinical expertise, optimizes effectiveness of patient education and engagement strategies to promote patient understanding and positive experience with early, proactive consideration of care preferences and care planning.
  • Participates in operational and clinical improvement initiatives that increase the effectiveness and efficiency of ACP processes across the care continuum
  • Provides education to care teams in order to improve patient, clinician and provider experience of advance care planning and serious illness care.
  • Collaborates with other TOC groups to embed ACP throughout system initiatives.
