About The Position

Program Manager-Dementia Guide Licensed Master of Social Worker LMSW, or Licensed Clinical Social Worker (LCSW) Full-Time The Parker Jewish Institute for Health Care and Rehabilitation, located in New Hyde Park, NY, is seeking a Dementia Guide Program Manager. In this position, the Program Manager is responsible for providing comprehensive psychosocial assessment, care planning, caregiver support, and resource coordination for patients enrolled in the CMS Guide Model. This role supports interdisciplinary dementia care management, ensures compliance with CMS model requirements, and promotes quality outcome for patients and caregivers. About Parker The Parker Jewish Institute for Health Care and Rehabilitation, conveniently located on the Queens-Nassau County border in New Hyde Park, New York, is a non-profit health care facility that offers inpatient programs such as sub-acute/short term rehabilitation, long-term care and nursing home care, as well as community health services encompassing certified home health care and a comprehensive community hospice program that serves terminally ill patients in their own homes or in nursing facilities, including Parker's nursing home. Requirements

  • Minimum two years of experience in geriatrics, dementia care, hospice, palliative care, or chronic care management preferred
  • Master's Degree in Social Work is required
  • New York State Social Work license, LMSW/LCSW
  • Knowledge of community resources and long-term services and supports
  • Experience with Medicare documentation and compliance preferred

Responsibilities

  • Conduct comprehensive psychosocial assessments for patients with dementia and their caregivers
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Provide ongoing care coordinator and follow-up per GUIDE Model requirements
  • Deliver caregiver education, counseling, and emotional support
  • Connect patients and families to appropriate community-based services and long-term supports
  • Monitor social determinants of health and address barriers to care
  • Assist with advanced care planning discussions when appropriate
  • Maintain accurate and timely documentation to meet CMS GUIDE Model standards
  • Participate in case conferences and interdisciplinary team meetings
  • Support quality reporting and performance metrics related to dementia care management
