About The Position

Under the direction of the Manager of Population Health, the Program Manager, Population Health works to develop, implement, and evaluate various Population Health Management (PHM) programs and initiatives to improve health outcomes. The Program Manager, Population Health uses data-driven approaches and facilitates cross-functional teams to develop population health interventions to reduce health disparities among HPSM’s member population.

Requirements

  • Bachelor’s degree in health care or related field experience required.
  • Two (2) years’ experience working in project management/program management or leading Population Health Management initiatives, preferably in a health care or managed care environment and experience planning health outcomes improvement interventions.
  • Advanced data analysis and summarization skills.
  • Advanced communication (written, verbal, presentations) skills.
  • Advanced facilitation and consulting skills.
  • Advanced proficiency in Microsoft Office Suite applications, including Outlook, Word, Excel, Access and PowerPoint.
  • Extensive knowledge of PHM and Health Outcomes Improvement concepts and terminology.
  • Extensive project management and program management.
  • Extensive quality improvement and evaluation methodology knowledge.
  • Ability to develop and maintain collaborative relationships.
  • Demonstrated ability to communicate effectively with various audiences and individuals of diverse backgrounds.
  • Demonstrated ability to work as part of a team and support team decisions.
  • Ability to adapt to changes in requirements/priorities.
  • Ability to think critically and make informed decisions.
  • Demonstrated ability to influence without direct authority.
  • Ability to stay current in subject matter expertise and required role skills.
  • Ability to travel in/to San Mateo, San Francisco, and Santa Clara counties (primarily) and up to Sacramento on occasion.

  • Having obtained or working towards a master’s degree in a related field preferred (e.g., MPH, MHA, etc.)

Responsibilities

  • Conduct ongoing population assessments using population level data and gap analyses to identify trends, needs and priorities impacting the HPSM member population.
  • Develop and implement tailored engagement strategies to facilitate collaboration among HPSM employees, vendors, and local and community partners to support PHM alignment with needs and organizational priorities.
  • Disseminate detailed information and provide actionable next steps based on the findings of the population needs assessment.
  • Serve as the primary HPSM liaison to meet regulatory MOU requirements (e.g., Local Health Department), including but not limited to meeting facilitation, prep, and follow-ups.
  • Facilitate the intervention process including the design, implementation and evaluation of initiatives and interventions to improve health outcomes or other identified gaps.
  • Facilitate the prioritization and action planning process for identified needs.
  • Facilitate cross-departmental workgroups intervention planning processes and implementation efforts in support of PHM priorities.
  • Conduct program evaluations using quality improvement methodology.
  • Track PHM program dashboards, author reports, create and update desktop procedures for ongoing processes, and conduct other administrative activities to support the success of Population Health programs.
  • Monitor PHM performance metrics and trends and recommend continuous improvements.
  • Maintain PHM program documents, including the Population Needs Assessment (PNA), PHM Program Description, annual PHM work plan, and ensure ongoing compliance with requirements and policy implementation guidance provided by regulatory entities.
  • Act as the PHM liaison to organizational initiatives and projects as needed.
  • Stay up to date with regulations on subpopulations and HPSM populations of focus.
  • Partner with the Manager of Population Health to ensure alignment organizational strategy and with other programs across HPSM.
  • Adhere to work performance norms and attendance expectations.
  • Perform other duties as assigned.
