About The Position

The Program Specialist I provides and coordinates individualized, person-centered supports for individuals with disabilities to help them live as independently as possible in their homes and communities. This role conducts in-depth assessments of each person’s needs, develops individualized service strategies, and ensures that appropriate resources and supports are in place. The Program Specialist I coordinates services across multiple areas including medical care, housing, social services, employment or day programs, recreation, and community engagement. The role focuses on developing, implementing, and monitoring Individual Support Plans (ISPs) to ensure the health, safety, and overall well-being of the individuals supported. The Program Specialist works collaboratively with staff, families, community providers, and other stakeholders to ensure individuals receive comprehensive services and opportunities to thrive.

Requirements

  • High School Diploma or equivalent required.
  • 3–5 years of experience working in the disabilities field or related human services setting.
  • Strong organizational and time-management skills.
  • Detail-oriented with strong documentation abilities.
  • Ability to work independently with minimal supervision.
  • Strong problem-solving and decision-making skills.
  • Collaborative mindset and ability to work effectively with teams.
  • Ability to develop and implement new services or procedures when needed.
  • Strong interpersonal and communication skills.
  • Maintain compliance with OSHA, HIPAA, and agency safety policies.
  • Attend required trainings and meetings.
  • Maintain accurate documentation of time worked and assigned tasks.
  • Follow all organizational policies and procedures.
  • Lift up to 50 pounds.
  • Sit, stand, walk, bend, and reach during the workday.
  • Drive to program locations and participant homes.
  • Conduct visits in community settings and residential homes.
  • Maintain awareness of and compliance with: Bloodborne Pathogen standards OSHA safety guidelines HIPAA confidentiality regulations Fire and safety procedures Right-to-Know regulations.

Responsibilities

  • Ensure residential services are delivered in a professional, respectful, and person-centered manner.
  • Ensure services comply with all applicable agency policies, licensing standards, and regulatory requirements.
  • Monitor program operations and evaluate staff performance related to care delivery.
  • Ensure adequate program supplies, equipment, and resources are available.
  • Make recommendations for program improvements when necessary.
  • Work closely with admissions and discharge teams to identify individuals needing supportive living services.
  • Ensure individuals supported receive adequate and appropriate services.
  • Coordinate care among internal staff and external providers.
  • Maintain accurate and confidential records for each person supported.
  • Communicate with healthcare professionals and service providers as needed.
  • Consult with family members, guardians, and other support networks to ensure continuity of care.
  • Provide person-centered support in the home and community.
  • Assist in identifying and managing home care needs, including: Personal hygiene Household management Daily living activities
  • Respond to unexpected situations or changing support needs.
  • Coordinate and schedule medical appointments and related services.
  • Ensure medical recommendations and treatment plans are followed.
  • Communicate medical instructions and updates to staff and caregivers.
  • Establish and maintain relationships with community service providers.
  • Assist individuals in accessing community resources and services.
  • Educate individuals, families, and support systems about available program services and community supports.
  • Serve as a liaison between individuals supported, staff, families, guardians, and external service providers.
  • Maintain strong communication between all parties involved in the support plan.
  • Ensure concerns and service needs are addressed promptly.
  • Maintain confidential files and documentation for individuals supported.
  • Prepare reports and documentation as required by the agency.
  • Participate in meetings, committees, and program planning activities.
  • Assist with resource allocation and program support.
  • Perform additional duties as assigned.
