About The Position

The Department of Medicine 's Division of Geriatrics is looking for a Program Supervisor for The Patricia Bauman Center for Integrated Care, a new interdisciplinary care model designed to improve outcomes for frail, older adults with multiple chronic conditions, cognitive impairment, and significant social determinants of health challenges. The Program Supervisor reports to the Medical Director, and works closely with clinical, finance, compliance, and development partners. The Program Supervisor will ensure the Center’s financial sustainability, operational efficiency, and strategic growth by overseeing administrative functions, optimizing billing and reimbursement pathways, coordinating grant and philanthropic initiatives, and supporting program evaluation and data-driven decision-making.

Requirements

  • Master’s degree in Public Health (MPH), Healthcare Administration, Public Administration, or related field.
  • Two -three years experience in healthcare program administration, preferably within geriatrics or chronic disease programs.
  • Knowledge of reimbursement mechanisms and billing workflows.
  • Grant writing and budget management experience.
  • Strong analytical skills with the ability to interpret clinical and operational data.
  • Excellent organizational, communication, and project management skills.

Nice To Haves

  • Experience with geriatric or dementia-care programs.
  • Prior experience in academic medicine or large health systems.
  • Familiarity with population health, QI, and interdisciplinary teamwork.
  • Knowledge of EPIC, Microsoft 365, Impact of the Role The Program Supervisor ensures that the Patricia Bauman Center achieves its mission to improve the wellbeing of frail older adults in the Bronx. By supporting efficient operations, sustainable financing, and strong data-driven performance, the Program Supervisor helps build a model of care that addresses both medical needs and the social determinants influencing 80% of health outcomes.

Responsibilities

  • Oversee day-to-day operations of the Center to ensure smooth interdisciplinary workflows.
  • Develop administrative structures that support integrated geriatric and dementia care.
  • Coordinate team meetings, case reviews, and clinical program logistics.
  • Monitor access, patient flow, staffing ratios, and bottlenecks; implement improvements.
  • Optimize use of new billing codes related to SDOH, care management, and dementia care.
  • Partner with finance and compliance to ensure accurate coding and reimbursement.
  • Track revenue, expenditures, and long-term sustainability metrics.
  • Support multi-year financial planning.
  • Lead grant-writing efforts to support Center operations and research activities.
  • Identify new federal, state, and foundation funding opportunities.
  • Partner with Development on donor engagement and reporting.
  • Ensure adherence to reporting requirements for funded initiatives.
  • Develop dashboards to evaluate clinical and operational performance.
  • Assist with research activities, data collection, and IRB coordination when needed.
  • Prepare annual program reports and philanthropic impact summaries.
  • Support recruitment, onboarding, and workflow development for key roles.
  • Maintain communication structures across the interdisciplinary team.
  • Help define productivity expectations and staffing frameworks.
